Załącznik nr 1 from your employer: how it is filled in MOS
Updated 24.10.2026
In Załącznik nr 1 (annex no. 1) your employer describes the terms on which they employ you. It is an annex to your residence card application. Since 27 April 2026 nobody downloads or prints a form for it. MOS emails your employer a link, they fill in and sign the annex online, and until your employer has signed, MOS won't let you send the application.
Who needs Załącznik nr 1
It is required for these permits:
- residence and work permit (pobyt czasowy i praca);
- Blue Card;
- a business permit if you work on the board of your own company (spółka) or as a commercial proxy (prokurent).
For studies a similar annex is filled in by the university, and for an internship or volunteering by the organiser, and it is sent the same way, through a link from MOS.
What your employer fills in
First your employer gives their own details, that is the company name or their name, address and register number, NIP and REGON, plus phone and email. Then come the terms of work.
- position and occupation code;
- place of work and type of contract;
- working hours and gross pay;
- duties and period of work.
Your employer also declares that they have not been penalised for violations when employing foreigners or for certain crimes. If you work through a temporary employment agency, the annex includes the details of both the agency and the company where you actually work.
Either way, the terms in the annex must match your contract and still be valid on the day the office issues its decision.
How it works in MOS
- While filling in the application, enter your employer's email address and click “Dodaj”.
- Fill in the rest of the application and click “Zapisz wniosek i prześlij formularz pracodawcy”.
- Your employer receives an email with a link to the form. The link is valid for 30 days.
- Your employer fills in the form and signs it through login.gov.pl, and the signature must come from a person authorised to act for the employer, using Profil Zaufany, podpis osobisty or a qualified electronic signature.
- Once the annex is signed, you receive the email “Wniosek jest już możliwy do złożenia”, and your application in “Moje dokumenty” shows the status “Wniosek możliwy do podpisu i wysłania”.
- Check the application, sign it and send it. Save the confirmation of submission (UPO).
Until your employer signs, the application shows the status “Oczekuje na wypełnienie załącznika przez pracodawcę”.
Check the application before sending it to your employer
As soon as you send the form to your employer, you can no longer edit the application, and if you find a mistake later, you will have to create a new one. So check all the details before you click the button.
By the way, your employer can't attach documents to their form, so you attach the contract and other papers to the application yourself.
How to prepare your employer
Talk to your employer before you send the form.
- Confirm the email address for the link and check it for typos.
- Ask whether the person who will sign the annex has Profil Zaufany, podpis osobisty or a qualified signature.
- Tell them the email will come from MOS and the link in it is valid for 30 days, and ask them to check the spam folder.
- Agree on when they will fill in the annex.
UdSC warns that waiting for the employer's signature can take a long time, so don't leave your application to the last days of your legal stay.
When you need a paper form
The paper Załącznik nr 1 form is still used to change a permit you already have (zmiana zezwolenia), for example when you change employer. Such an application is submitted on paper. The form is on the MOS website, in the “Wniosek” section.
How Ekspat helps
If your application is based on work, the Ekspat questionnaire asks for your employer's email address. Ekspat fills in the application in MOS. Your employer fills in and signs the annex itself, through the link from MOS.
